Submission Requirements:
- Acord 125 & 127
- Owner's Name, DOB & Home address
- Contact name & email
- Loss runs (new in biz -ok)
Ineligible risks include: Vehicles rented or leased to others, Public Administration, Educational Organizations, Driving schools, Fuel Haulers, Social Services, Auto Dealers, General Freight Delivery, Time Sensitive Delivery, Sand & Gravel hauling, Refuse Handling, Dumps and Mix In Transit.
